Your child’s school is participating in the “Safe Routes to School” program. The goal of this program is to encourage and enable students to walk or bicycle to/from school on safer routes. These questions will help us learn more about children walking or biking to school. Your participation in this research study is completely voluntary and you may refuse to answer any particular questions. This survey should take you approximately 15 minutes to complete. By volunteering to complete the survey and returning it to your child’s school and then to Michigan State University, you indicate your consent to participate in the survey. However, if you choose not to participate, you will not suffer any penalty. There are no known risks associated with your participation in this study. Your participation in this study may contribute to a better understanding of issues concerning motorized and nonmotorized transportation to schools. Your child will be asked to fill out a survey at school. You can request a copy of this two-page survey for review from the principal. If you would rather your child not participate, please contact the school’s principal.
